complete startup/shutdown time info #93

Per request from @bknueven, clarify the startup/shutdown time information in gen.csv

@danielolsen, per your email comment, the RTS-96 Table 10 information suggests that we had mis-used and mis-labeled startup times.

The RTS-96 data has startup times where hot<cold<warm. I cannot fathom the logic in this, so I'm keeping the current RTS-GMLC convention where hot<warm<cold startup times. I'm also adjusting the definition of the columns in the README.md
