Update map, change mandatory form fields

Added new fields for map pop-up

Altered mandatory GroMoPo fields (note, I just changed the *, did not do anything else).

No longer mandatory:

• Model developer primary email *
• Is the model developer's institute located in the same country as the model location? *

Now mandatory:

• Link to paper, report, or model
• Brief Descriptive Model Name (i.e., Edwards Aquifer model)
