Attendee report tweaks #7

Attendee report might not need address, since it will often be blank (whenever they specified home address rather than work). Also the report could include company name.

I added company to the report and removed address fields. As you point out, address could be in either Work or Other (Home) so we don't want to have two sets of address fields on the report.
