Modify landing experience

Current website displays static data out of spreadsheet on landing page. This is intended to be changed with this issue. It should,

first ask for which city user is looking to get data for. List of cities may be made hard-coded or can be dynamically populated from data available.
second ask for what type of data needs to present. Currently website displays following type of datasets:

Remedesivir, Plasma Support, RT-PCR, Oxygen, Covid Care Centers, Food, HomeCare/Doctor on Call, Ambulance, Emergency Information, Useful Websites

Here is a good inspiration on how a landing page could look like - Sample. In this case it could be,

I want information on Covid Care Centers in Ahmedabad.

Based on above search criteria website should be able to render data which is complying that service from asked city.
