Add option to automatically create note types #52

Currently the note types are available as templates only which is obviously confusing (#46 #50 #51). Add an option in the menu that defaults true to automatically create these.

We could also maybe take this as an opportunity to bring the code which constructs the menu up to date. I kind of mentioned it at the end of #42.
