Some code quality changes #1

These were recommended in an internal code review from Signify.

Note that there are still some markdown linting "issues" - namely repeated headings ("Usage" and "Known Issues") which are intentionally repeated, and the use of a second top-level heading in 02-extensions.md which is also intended.
