I've fetched the NOSA in plain text from https://opensource.gsfc.nasa.gov/nosa.php and edited it to add the specifics for Cfitsio. I've added its most recent NTR reference number dating from 2020. I also moved it into the licenses subdirectory. In principle, contributors could add code that is subject to a different license (see NOSA section 3.D) so a top-level licenses directory is a good practice for handling the possibility. (See astropy for instance.)
