Provide a mockup for surveys in LenaSYS

We want the ability to:

• Perform surveys in our courses, for example as a complement to other course evaluations

• Allow students to create surveys in courses

This is the page for participants. Question one demonstrates what it looks like when you have checked a box by clicking the awnser. Press hand in all to save all answers you have checked so far and exit the page to back where you came from. Doing the quiz again overwrites your old answers. If you are logged in as the quiz maker or superuser then you will se a R symbol on the navbar. Click this to open a textbox where all the current answers are written.

This is an alternative to what the form could look like when creating one. In the first box, the creator selects the name of the form and a description. Then questions can be created using the plus icon on the right menu page. Through the design dropdown menu in the question box, you can choose between making multiple choice options, check boxes and short answers. If you want to delete an alternative to a question, this is done through the cross assigned to the alternative. To delete the entire question, click on the trash can icon located in the question. To add a title, use the icon located in the menu on the right in the same place, a section can also be added.
