Add support for ClinicalDocument/sdtc:statusCode #18

Came up as a use case through Social Determinants of Health (SDoH). Add support for ClinicalDocument/sdtc:statusCode

Suggested place for rendering is in the document title. Example in Dutch for English "Status: completed"

As suggested in #21 I have moved the sdtc:statusCode from the document title into the document header data that is rendered towards the bottom of the output. Screenshot (red square just to draw attention in the screenshot, not part of the html):

@LisaRNelson could you confirm if this suits the use case?
