discussed in meeting:

  1. The extension should be a modifier extension
  2. Alternative would leverage the combining rules to permit patient access, then another rule deny access to tagged elements. Would still need an extension, but may be more clear.

I think this has all use-case analysis included.

not true. you just need to tag the value, not the extension

reviewed today. Once the REL extension on extension fix is inx, agreed to merge.
