Update TOC FHIR artifacts to match fhir artifacts page #504

Ideally group and sort examples and fhir artifacts to match the fhir artifacts pages and link the group headers to the FHIR artifacts page anchors

Will look at the possibility of doing this as part of the revamped artifacts list (driven by Composition). Can't do it with the artifacts list as it is because there are no pages for the different categories of artifacts and the ToC is driven off page hierarchy.
