Spelled out SHC early.
Added bullets to spell out AD and DM and say a little more about the parent-child relationships and constraints.
Changed SHCPatientUnitedStatesDM to SHCPatientUnitedStatesAD in the 4th item in 1.5 which starts "Cardinality constraints are applied to specific data elements in Allowable Data profiles ...", so I think AD makes more sense rather than DM.
