FHIR-33380 and FHIR-33381 #183

https://jira.hl7.org/browse/FHIR-33380 and https://jira.hl7.org/browse/FHIR-33381 Both are from Keith Salzman 33380 is about spelling out acronyms like SHC and SHCPatientUnitedStatesDM Spelled out SHC early. Added bullets to spell out AD and DM and say a little more about the parent-child relationships and constraints.

Changed SHCPatientUnitedStatesDM to SHCPatientUnitedStatesAD in the 4th item in 1.5 which starts "Cardinality constraints are applied to specific data elements in Allowable Data profiles ...", so I think AD makes more sense rather than DM.

33381 is more focused: FDA and EUA.
