Scope and relationship with parent SMART Health Cards spec
Clarify scope of IG on index.html based on updates to the SMART Health Cards spec. The SHC spec now defines the specific use case that this IG covers, and removes SHC types like https://smarthealth.cards#covid19 in favor of filtering based on relevant FHIR data elements (e.g., filtering by Immunization.vaccineCode to identify all the COVID vaccine SHCs). The latter is supported by enhanced, community-maintained ValueSets on https://terminology.smarthealth.cards, which this IG also now takes advantage of.
Update profiles.html to reflect the changes described above.
Vaccine terminology
Remove GTIN, AIR, and ATC as options for Immunization.vaccineCode. Now only CVX, ICD-11 MMS, and SNOMED CT are allowed for identifying vaccines.
Update Immunization.vaccineCode slice bindings for CVX, ICD-11 MMS, and SNOMED CT to use ValueSets from https://terminology.smarthealth.cards, which is a community-maintained resource that can be automatically/ quickly updated from the source of truth code systems published by CDC, WHO, and SNOMED Int'l respectively.
Remove Immunization.manufacturer from DM profile given criteria that the value of Immunization.vaccineCode SHALL be specific enough to allow for evaluation of compliance to the vaccine schedule in the jurisdiction of the Issuer, making Immunization.manufacturer unnecessary for vaccine specificity. The MS for this element was also removed from the AD profile.
Rename monkeypox to mpox throughout.
Remove vaccine-related ValueSets from the IG in favor of using ValueSets from https://terminology.smarthealth.cards that can be kept up-to-date outside the HL7 IG publication workflow.
Update vaccination.html to reflect the changes described above. This includes adding in better explanation for which subset of the above code systems can be used to identify vaccines, and where to find the source of truth for each of these code systems.
Lab test terminology
Remove vc-shall-not-be-a-covid-loinc invariant - this was causing false positive errors, likely due to terminology.smarthealth.cards binding.
Fix generic lab results value set to include SNOMED CT descendants of 441742003 "Evaluation finding (finding)" or 362981000 "Qualifier value (qualifier value)" -- previously this was just set to descendants of 404684003 "Clinical finding (finding)", which was not correct.

Other changes
Cleaned up ignoreWarnings.txt to reflect the current version of the IG Publisher.
Minor updates to profiles.html for clarity.
Minor updates to the use cases narrative on index.html to be more contemporary.
Removed "Compatibility with IIS" section on index.html as this does not seem to be relevant anymore.
