Add table captions and anchors #120

New formatting for table captions

Section 3.6.9 Flag Rules table is not captioned, and section 3.7 Appendix is not captioned. Not sure if those were intentional.

Fixed the Flag table. I don't think the table in the appendix needs a table number since the section heading serves that purpose.
