split up page-end #186

The page end template includes this fragment:

based on FHIR {{site.data.fhir.version}}

(Currently on line 71)

it would be good to split the line that contains this out into a separate include so that the UTG template could vary this part without replaceing the entire page-end template

I'm not sure. All of the XML, JSON, etc. instances available from the HTO site are in fact tied to a specific version of FHIR. I think that needs to be highlighted somewhere, and a tiny notation at the bottom of the footer that's consistent with the way we do it everywhere else seems like a reasonable and easily justifiable option.

HO is special, and in fact there are multiple distributions for different versions of FHIR. So the statement needs to be changed for HTO. As it is, we've had committees refuse to use HTO in R5 because it's for R4 not R5
