Is your feature request related to a problem? Please describe. When codes are used in CDA documents their CodeSystem is usually (at least in Austria for the ELGA documents) represented as OID. In FHIR however, they are represented as URLs. The according ValueSets for both standards are managed and maintained on the Austrian terminology server TerminoloGit. It provides the ValueSets amongst other formats also in a FHIR. For implementers, that are dealing with both standards, it would be great if the FHIR representation of the ValueSets could contain the OIDs for the CodeSystems as well. This would avoid using mapping mechanisms or lookup operations.

Describe the solution you'd like Creation of a profile in the HL7 Austria FHIR Core IG on ValueSet to add the alternative representation of CodeSystems as OID for the used codes.

Describe alternatives you've considered

• The FHIR $lookup operation
• Using a mapping file
• Using alternative format from TerminoloGit (they exist but are already marked as deprecated)

Additional context This profile might be relevant for other countries as well that are using CDA guidelines.

During the TC FHIR call on December 15th 2022 multiple options on how to implement this feature request were discussed. The decision was to create an Extension for ValueSet.
