Use only one column for appendices
- Appendices in a separate file
Fix a non itemized glossary entry (Concurency Forum)
- Also affects csbs-arxiv

Damn, I did it again and merged a WIP (so annoying that github lets you do this). Anyway, maybe this makes it easier to pull in fixes a few at a time...

Don't worry, what was there was minimalistic but ready for merging! I thought GH was not allowing the merge when the topic starts with [WIP] like GitLab but as a repo admin you may bypass it (may be too easily).

You'd think it would at least warn you on merging a WIP. GitLab just won't let you do it at all.
