Fix PERRT display as per new structure #173

Current plan is to simplify and split

vital signs for relevant patients
perrt referrals for relevant patients
bed status/location comes instead from the census API

rather than everything together. The front end can then assemble these as needed The vitals endpoints can be used to drill down into individual patients.
