Where: If player dies after completing the Holodeck sequence
When: When clicking on the "Last Save" on the Mission failure menu screen
This is probably due to the autosaving not ovewriting the last autosave but rather creating a save with a dedicated name.
The load last save appears to load only the latest auto/quick save.

In the changelog these two new saves have been added and are to blame.
ADDED: map1, Automatic Save Game at level start
