Add cvar to allow disabling respawning at where the player died, so the player has to go all the way from spawnlocation as a kind of extra difficulty for players that just like torture.
Ask Suxev why he likes that kind of punishment, but I can see others liking it, so we should add it and make it votable on the fly.
