As a Data Analyst I want to have consistent town names and corresponding GPS locations So that I can effectively analyze the spatial distribution of patients

Acceptance Criteria:

The following needs to be completed for this story to be "done":

[ ] I can type in a town name and see potential choices (That have GPS data)
[ ] I can then select any of those potential choices as the input I want to the patient form
[ ] I can type in a new town name if I see no correct matches

We only want to store town names and the GPS data once in the database
