Remove reset button from surveys #43

In the discussion of on boarding there seemed to be a consensus that we remove the reset button.

just for mobile resolutions or throughout? both will take but a second to do

Everywhere...both mobile and desktop. When done, please tell me what file and block of code you updated.

ok updated line 120, 121 and 282 on survey.css

The latest updates to survey.css also incorporate the fix for #2
