File management, ACLs, decouple doc title from file title #24

This is a temporary holding issue as a number of different questions are bundled up here.

Important is translation; forking.

Also need to look at the conventions for file storage: I'll consult Darron Broad; maybe others.

It could be a good opportunity to put out a limited RFC.

File management - A per publication data management directory structure is needed. The structure needs to have features for: releases, versioning, forks for example for associated translations or copy/duplications, for archiving, and as an optional feature if possible as Git submodule reuse in another publication.

ACLs - user groups are needed to give a number of users access to a whole group or single: files, directories, and sub directories for publications. ACLs are also needed for templates and themes.

Decoupling doc name from doc title - Is it possible to decouple the document titles from the document name in Fidus Writer. Document management becomes impossible when you have 100 documents called: Abstracts, Introductions, Conclusions, etc.

User moderation - New user accounts need to be moderated by an administrator role, sending messages to the admin and to the user. New users need to be added to publication user groups. An account suspension function is needed with associated messaging.
