This should make #1238 easy.

[x] Revert the hacky logicals -> numerics fix in explore (in master) once this is done, since they'll get prepped.
[x] Deprecate ready_no_prep and determine_prep
[x] Change the no-prep tests in predict to prep_data(no_prep = TRUE)
[x] Add error if prepped_df passed to predict(newdata)
