Fix off by 1 page index entries

Currently, some of the index entries are off by one page. The correct ordering of latex commands to fix them:

• lua/pdflatex twice
• upendex
• lua/pdflatex once

No latexmk for printable build. This makes the printable build step run in less than one minute.
