Fixes and improvements #262

This PR has the following fixes and stability improvements: -- Fix BundleValidator check causing "inconsistent balances" error under load -- Rescan tips and evict pointers that are not hashes anymore -- BoundedQueue has adaptive eviction policy to mitigate overflows under high load
