Subsequent Application has trouble when there are 3 app dates #1742

Scenario is that there is a case with 3 separate applications. This should only happen (when correct) if a case is pending for HC, CCAP, and cash/food programs with all 3 different application dates.

The current functionality only supports 1 additional application date and does not consider if it was a CCAP request or not.

This needs to be rushed because this is a rare situation and we have a testing case - see team email from HA.

This seems to be working and we did not bread application received.
