To support the new interview team, we need verbiage at the end of the interview script that can be read directly for the form reviews.

These workers will have less familiarity with the details and policies surrounding these forms and additional guidance is needed.

Still determining if we can use the paragraphs below or if we need more information for additional forms (more detailed bullet points).

FORMS/HANDOUTS FOR APPLICANTS (state.mn.us) specifies the following:

Responsibilities and Rights You are responsible for reporting changes that might impact benefit eligibility such as employment, income, property, household status, citizenship/immigration status, address, expenses, etc.). For cash/child care assistance, report changes within 10 days of the change. For SNAP, report changes by the 10th of the following month. For SNAP 6-month reporting, only report if income exceeds 130% FPG or if ABAWD and work hours drop below 20 hrs/week. Change reporters must report changes in source of income, change of over $125/month in gross earned income or unearned income, unit composition, residence, housing expense, child support, and if ABAWD and work hours drop below 20 hrs/week. For Monthly reporting, submit the Household Report Form every month with income and change verifications attached.

Providing false or incomplete information can lead to loss of benefits/criminal charges. Agencies may verify your information, requiring your consent. Using your benefits acknowledges that you've reported any changes. For child care, you may need to pay a co-payment, additional costs, or provide children's immigration/citizenship documentation; failure to pay or cooperate may end your assistance.

You have the right to privacy of your information, reapply anytime, receive a copy of your application, know why applications are delayed, know program rules, live where/with whom you choose, and report expenses. For SNAP appeals, you have 90 days to appeal. For Cash/Child Care appeals, appeal within 30 days of receiving notice. Free legal services are available. Discrimination is illegal; if mistreated by a human service agency, file a complaint.
