CASE NOTE script that will detail the specifics of how the income calculations were completed, this includes:

• Cost Effectiveness/Affordability
• 200% Income Limit
• Household Assets
• Household Income after application

This script could also review the detail of the emergency amount:

• Total amount of emergency
• Line item breakdown of the emergency amount (late fees., other fees, past due amounts, etc.)
• Amounts paid by other agencies or people
