Renames BadMedicine to SynthEHR

Update Plan:

Publish SynthEHR v2.0.0
Update Repo name
Update all downstream dependants to use renamed package

@JFriel Looking good, we should first push one 'final' release under the existing name with a deprecation warning/message saying to look for the new name though.
