Duo Authentication not working #831

What steps will reproduce the problem?
1. Login to Duo.
2. Duo-two factor prompt is pushed out to phone.
3. The Duo icon is pressed on the Dashclock widget

What is the expected output? What do you see instead?

Expected output: Duo is authenticated

Actual output: 
Invalid Request
Unable to view request at this time.

What version of the product are you using?

1.6

Which device and Android version are you experiencing this issue on?

Device: Sprint LG G3
Android: 4.4.2
