Add meaningful descriptions to created LUNs #42

Currently all newly created LUNs get description: "Created from Kubernetes CSI" This is not meaningful. It would be helpful if the description contained some actual information on the LUN, e.g. cluster name, namespace and name of PVC that triggered the creation of a LUN.

The "description" parameter can be customized. For details, see the user guide.
