Make Page Title and Page Slug required.

Page slug being required is for another improvement that will be made to allow creation of a functions to get field values based on the page-slug when the settings are to save value to the options page adder post.

Additionally, I will need to require that the options page slug is unique across all options pages added with this plugin.
