Load CDR 2016 to Factotum

• [x] Add index for chemical names
• [x] Create new source in Factotum for CDR and new group for the 2016 report, and load all four csvs to Factotum
• [x] Extract data
• For Consumer and Commercial Use:
  • Include columns AQ-AS in extracted data
• For Industrial Processing and Use:
  • Include columns AQ-AS in extracted data
• The other two csvs do not need to be extracted, but they should be in Factotum
• Put column A in cpcat code
• [ ] Upload extracted data to Factotum
