REFACTORING: Distinct folders for app and proxy. #91

New folder structure:

to be more intelligible,
to avoid pushing unrelated (and possibly sensitive) files with couchapp,
to prepare future deployment process (based on docker).

I made you the author of the first commit, because otherwise my name would appear on every file that I have just moved (making people think I am the author, while I am not).
