Isanteplus forms rendering issue #52

What we know:

  1. When booted up with recent versions of dockerized isanteplus-server and isanteplus-db, the forms take forever or fail to load when selected in an active visit. Behavior does not change when MySQL image is simplified, modified, streaming pipeline is off, etc.
  2. For previous versions of the images, the issue did not happen. It could have popped up since the recent modifications to the MySQL image that used the split sql files.
  3. The same version of the isanteplus-server image works fine with an AWS RDS MySQL instance.

To test:

  1. manually run db ipdate scripts on running MySQL instance
  2. Load previous version of db, and new concepts on top of that.
  3. Run off of AWS
