We need a new license document for CiC in hotcrp. #57

When a final paper is uploaded, the authors must also complete a license form. We have been using a form for transactions that is pretty close, but may need revision. The current license form is an HTML template. I recommend that we copy that one and create a new one called cic_agreement.html with wording that is specific to Communications in Cryptology. A clumsier way to do it is to edit the the journal_copyright.html document so that it's generically for any journal, but that's clumsy since it references Ruhr University. This would go into the github clone of hotcrp.

This is now fixed with this commit and the form that @jwbos submitted. I tested it on cictest2 and it entered the form into the database and updated the copyright flag for the paper.
