Pre-conference survey construction (phase 1)

@wibeasley, i put all the three surveys on a single form. I emailed you and @andkov the pdf. Here are a few notes:

• No question has been replaced or changed.
• Branching logic has not been applied to any of the questions yet.
• Do i need to look at every question carefully and see if a possible revision is needed?
• I am waiting on Andrey for the final decision on the .csv and further instruction from Dr. Beasley on the PCS.

I have incoorporated details from @andkov Model space map into the pre-conference survey (PCS). Here are few notes:

• Studies have been updated
• Only cognitive domains and constructs (outcome variables) have been added.

• Did you want me to use row labels or cell labels for the options in the following question:

  What variables do you collect associated with the cognitive construct?

For the record, @Maleeha is changing some the checkboxes (attached to each variable) to free text fields. This will allow the participant to enter the exact name of the variable (associated with the measure) in their dataset
