Add Organisation Registration Agencies as a codelist

• [x] Update metadata now that this is in the SSOT
• [x] public-database should be an XML boolean? (I'm also considering making it an attribute, instead of an element, as this is the easiest way to enforce that it only appears once).
• [x] website should be the same element name as used in https://github.com/IATI/IATI-Codelists/issues/4 (currently url is proposed)
• [x] Is the country element redundant? (Since this information is encoded in the first two letters of the code)
• [x] Should iati-usage-rules use the description element for consistency with other codelists?
• [x] Add extra fields to the codelist schema

What is wrong with redundancy with country? If you want to search on country it is easier to have a field available instead of splitsing code or searching at a start or within a string.

Good point.

Part of the reason for removing country was to avoid adding too many new elements that other codelists did not have. However, other codelists do have a category element, which is appropriate for the country code, so this is what I've used.
