Make the docs generate #292

There were a number of errors and a missing file that mean the docs were not building. This makes the changes required for the docs to build.

There are still a couple of warnings, though at least the docs build.

On a separate note, would a travis build stage (on the master and dev branches) be an appropriate way to test everything would be packaged up correctly? - i.e. this process: https://github.com/IATI/pyIATI/blame/f00cc1cfadc779abc771e528515c11fa0b0298bd/RELEASING.md#L26-L29 (Running python setup.py sdist would replace step 3.1).

Potentially, though more reading and experimentation would need doing in this area to confirm!
