Added a troubleshooting section that includes a fix for an issue I ran into last week. With the Blockchain Platform Extension for VSCode's version update, the default ports used for peer/orderer/CA were changed. Anybody using the newer version of the extension (1.0.4+) will therefore have to update the ports in the connection.json file in order to run the app successfully.
