I agree it looks jarring when IBM Plex Mono is used like this.
But as you already have noticed not all monospaced fonts have equal weight underlines. Especially for a family with a large range of weights such as Plex Mono it would be jarring if all eight weights (from Thin to Bold) would have the same underline thickness. In that case the underline stroke for Thin, ExtraLight and Light styles would not harmonise very well anymore.

I have one nitpick with the mono font: the underlines for bold and normal text have different weights. This looks very jarring when a color scheme uses both bold and normal text, and code is underlined:
From my experience, most monospaced fonts have equal weight underlines:
Iosevka, Menlo, JetBrains
