Update Deploy to Bluemix URL #14

The hub.jazz.net/deploy page is going away soon. Its replacement is https://bluemix.net/deploy which uses DevOps Toolchains to deploy the application.

This pull request updates the URL in the README file.

Sorry about that: this pull request was originally opened from a shell script and apparently it had some bugs.

I am going to reopen this because I don't know if this is pulled in first. @ibmkendrick can you fix this up and pull this in for us?
