It seems like the pypi isn't updated. When installing the Plugiin i get the following ImportError: cannot import name 'NetBoxModelCSVForm' from 'netbox.forms' (/opt/netbox/netbox/netbox/forms/init.py)

That is correct, this will be included in version 1.4. The release will be made when #8 is finished
