@mikeymckay - I have a few questions about this that you could probably answer in 10 minutes. I'm not quite sure how to establish the linking ids:

Program id Malaria Case Entity id Case id Attribute id Age Attribute id

I've created all of the above in dhis2.zanzibar.ictedge.org. There are ids in the JSON export of metadata for the above. Are those the ids needed above?
