Cases that have previously been accepted keep being offered.

There were some broken assumptions in how case acceptance was working. Instead of replicating ussd notifications to the tablet, editing it, then replicating back, now we just edit the doc on the server, create a local Case Notification and then replicate that back to the server. Also a lot of work was done refactoring away from callbacks.
