Improve user report calculations #64

  1. The issue with calculations on the summary reports still an issue. DMSOs claims to have categorize some of the cases as LOST TO FOLLOW-UP but when they are judged their performance those lost to follow-up still remains in the denominator and makes their performance low. maybe they need to taken off the denominator if are lost to follow-up. it will be the same thing as how it should behave when we finish linking index cases from the same household
