Track module completion #15

In each module, add variables to track the completion rate, e.g. number of blocks available vs number of blocks accessed

Edited module template https://docs.google.com/spreadsheets/d/1hcH8pFdiHZN0UvZgyv3Zht9ARBTx-VXhNBI2o8L7fHU/edit#gid=1285396468 variables ntb ID (number total blocks) ncb ID (number completed blocks) added a "review" column in "content_flow_structure" so that the variables are not added when the module is accessed in "review" mode and therefore they are not overwritten
