Clarify NRB specs and add open issues #22

Resolve issue #21 by calrifying NRB stuff. Also add some more open issues inline, move Custom Options to be under Options, and move the appendices for EPB flag word and assigned Block Types to the main body. (normative things such as assigned codes don't go in appendices in IETF docs, afaik) I put the Block Types in the IANA section for now, under the presumption they will become the initial values for an IANA registry. (another open issue :)
