Create an interface for alert details The interface should contain the following input/information fields.

• Alert ID
• List of related cases with case status
• Alert status
• Health risk
• Open text field
• Inform Data Collector button (Yes/No)
• Optional text field for DC feedback message
• Escalation button (Yes/No)
